Dog License
This license is issued by the City Clerk/Treasurer and expires December 31 regardless of the date issued. Any dog 5 months of age or older must be licensed in the municipality where the dog is kept. You must have proof of rabies vaccination before a license can be issued. You will be charged a late fee if the license is obtained after April 1.
Operator (Bartender) License
This is an annual license. All license, regardless of the date issued, expire June 30 each year. Complete and submit the form along with the fee to the Shullsburg City Clerk/Treasurer. Download Here
Zoning Permit
A zoning permit is required for new construction, remodeling, additions, renovations, driveways, accessory buildings, demolitions,roof, siding or for any improvement over $500. Replacing carpet and floors does not require a permit. Permit fee required when application is submitted. Building Permit

Certificate of Appropriateness (Historic District Only)
Required for new construction, remodeling, additions, renovations, driveways, accessory buildings, demolitions regardless of cost. Variance Appeal
An appeal may be filed with the Board of Appeals when is it allegd there is an error in any order, requirement, decision, or determination made by the Building Inspector/Zoning Administrator. Download Here
